/trust and /untrust are state-aware: the palette shows only the action available for the current workspace. /untrust requires an existing trusted decision. Skills appear as /skill-name when enabled. Prompt templates appear as /name (filter with resources.disabledPrompts or resources.prompts ! excludes).
